Dear Parents/Guardians:
Greetings! We believe our Media Center/Library is a vital part of your student’s success. Reading material in a variety of subjects and reading levels are available for check out. Your child will be instructed on how to properly care for and use the books within the school Media Center.
Due to an overwhelming number of books that have been lost, damaged, taken without proper check out, left in bathrooms, classrooms and outdoors students in grades 3 thru 8 will be allowed to check out two books at a time for a two-week period. 2nd grade will be able to check out one book at a time.
Checking out Books
- When a book is removed from the library shelf, students are not to re-shelve it. Instead, books
not being checked out should be placed on the cart for media assistants to reshelve.
- Students are encouraged to return books as soon as they are finished so others may enjoy them.
- Students with an overdue or lost book may not check out additional books until the late book is
returned or the lost book paid for or replaced.
- Students are not to write, draw, tear pages, or remove labels with barcodes from Library Books
- Young Adult (YA) content
- 5th grade students may check out Young Adult (YA) books which are suitable for older readers due to topic, content, language, and/or themes. Grades lower than 5th, are not permitted to check out these type books. If you do not wish for your child (in any grade) to check out YA books, please indicate this on the form. If your child brings home a book that is not labeled YA and you believe it should be, please bring it to the attention of Mrs. Bunch.
- Lost/Damaged Books
If a book is lost or damaged, the media center should be notified immediately. The student will be charged the cost of replacing the book. The student’s library privileges will be suspended until the matter is rectified. Refunds for lost books will only be given if the book is returned in good condition. Students should not attempt to repair the book. It should be brought to the Media Center “as is’’ so that Mrs. Bunch can assess and hopefully repair it. Students may not check out a book for another student for any reason. Doing so will result in library privileges being restricted.
- Book Return
Books are to be returned via the Book Return Bin located just inside the library entrance. This can be done anytime during school hours. A student should not wait until their designated library time but should return a book to the bin as soon as he/she is done reading it and before entering the library. Books must not be removed from the library without first being checked out. All students have library time once or twice a week and are encouraged to only check out books they can read within that time frame. To prevent books from being re-shelved without proper check in, a checked-out book should not be brought back into the library unless it is being renewed or being given directly to Mrs. Bunch.

- Library Etiquette
- Gum, food, or drink are not allowed in the library. Only a water bottle with water is allowed.
Water bottles are to be kept at the Hydration station upon entering the Library.
- Library time is quiet time, and the time is best used constructively. It is a time to read silently
and/or prepare for an upcoming class, individually. It is not a time for group work as this should
be done in the assigned class.
- Unless part of the instruction, students are to remain in their designated seat.
- Horse play and running is never permitted in the Library.
Remember, checking a book out from the library is a privilege and a huge responsibility. Privileges may be restricted or revoked due to abuse of the library rules, repeated overdue, lost, or damaged books. Students should be respectful of the books and return them in good condition so that others may enjoy them as well. Please sign and return this form to begin your library privileges. No books will be checked out until this form is returned. This contract will be valid for the 2023/2024 school year.
- As a student of Sidener Academy, I am willing to show responsible behavior in my actions in the Media Center and for the books I check out. I understand that failure to keep this agreement to the best of my ability may cause me to forfeit my privilege to check out books. I also understand that I must pay for or replace any lost or damaged books before I can check out new ones.

As the parent/guardian of a Sidener Academy student, I will help my child be responsible for the books he/she checks out of the Media Center. In the event that a book is lost or damaged, I acknowledge that I will work with my child to ensure that the cost of repair or replacement of the book is made in a timely manner.
